You’ve never heard of SOURCE or CCSP and are wrestling with all of the new changes in your loved one’s life. This website was created to help guide you through this process and answer many of the questions you have about personal care homes.

We are Georgia Health Services Network, a licensed Medicaid Provider in the State of Georgia. We work closely with families and the medical community (Social Workers, Case Managers, RN’s, etc) in finding affordable housing for individuals from the low and middle-income groups in need of health services. We are contracted with licensed personal care homes throughout Georgia to provide assisted living services (ALS) through the SOURCE and CCSP Programs.

Our cliental group is composed primarily of elderly and disabled persons who do not meet the level of care needed to live in a nursing home and for whatever reason, are unable or unwilling to live alone.

Typically, prospective residents of personal care homes need assistance with activities of daily living such as bathing and dressing, monitoring of their medications, meal preparation, laundry and housekeeping services, and supervision. Others simply require the security and socialization provided by living among other people.

On this website we try to explain what personal care homes do for residents, how one goes about paying for personal care home services, and what programs are available to assist in paying for these services. We encourage people considering the “personal care home option” to explore this website and use Georgia Health Services Network to assist them in obtaining placement in a personal care home.

As a licensed Medicaid Provider, Georgia Health Services Network does not charge any fees to families or individuals seeking placement in a licensed personal care home. Georgia Health Services also makes the application for residents to the two primary Medicaid programs (SOURCE & CCSP) at no charge; nor do we ever charge a placement fee.
